Job description

Our client is seeking a compassionate, forward-thinking Consultant Physician in Geriatric Medicine to join our dedicated multidisciplinary team. You will play a pivotal role in delivering comprehensive, person-centered care across our acute geriatric wards, specialized outpatient clinics, and community-reaching Older Persons Services.

Key Responsibilities

  • Clinical Care: Provide consultant-level leadership in acute frailty assessment, comprehensive geriatric assessment (CGA), and inpatient management.

  • Service Development: Collaborate with multidisciplinary teams (MDTs) to enhance integrated care pathways, rapid access frailty clinics, and hospital-at-home initiatives.

  • Multidisciplinary Leadership: Lead daily ward rounds, MDT meetings, and discharge planning to ensure seamless transitions of care.

  • Education & Mentorship: Supervise and train junior doctors, medical students, and advanced nurse practitioners.

  • Quality Improvement: Drive clinical audits, research, and governance frameworks to elevate patient outcomes.

The commitment will be 80% to a newly developed 110 bedded Older Persons in-patient unit located in Letterkenny, Co. Donegal and 20% integrated care activity to Letterkenny University Hospital which is located adjacent to the new Older Persons Unit. This arrangment will ensure strong integration between community and acute geriatric services.

The new unit will comprise of 50 short stay beds including rehabilitation, respite and step-down care from the acute hospital, it also includes a ten bedded dementia unit and 50 long stay residential beds and will open on a phased basis from quarter four 2026.

The post holder will lead the development of intergrated pathways of care for older persons accross the community, residential and acute care settings and will work with the existing Consultant Geriatricians in the acute setting to provide high quallity, person-centred, evidence based healthcare to older people in Co. Donegal.

Candidate Requirements

  • Qualifications: Specialist Registration (or eligible within 6 months) in the specialty of Geriatric and General (Internal) Medicine with the Medical Council of Ireland.

  • Clinical Expertise: Proven experience in acute frailty management, orthogeriatrics, stroke care, or memory services.

  • Skills: Outstanding communication, team leadership, and holistic clinical decision-making skills.
