You will lead multidisciplinary clinical teams in providing Comprehensive Geriatric Assessment (CGA), developing subspecialty clinics, and bridging acute and integrated care hub services.
Key Responsibilities & Duties
Clinical Service Delivery: Provide consultant-led inpatient care, acute admissions management, and specialty ward rounds for older adults, ensuring timely Comprehensive Geriatric Assessment (CGA) and individualized care plans.
Ambulatory & Specialist Clinics: Lead outpatient clinics, including Rapid Access Frailty Clinics, Falls & Syncope Clinics, Memory Assessment / Dementia Care, and Movement Disorder services.
Integrated Care & Outreach: Collaborate closely with the Specialist Geriatric Teams (SGTs), Community Health Networks (CHNs), and Integrated Care Programme for Older Persons (ICPOP) hubs to facilitate early supported discharge and prevent avoidable emergency admissions.
Liaison Geriatrics: Provide specialist input to inpatient liaison services according to service configuration (e.g., Orthogeriatric liaison, perioperative older person care, or stroke rehabilitation).
On-Call Commitment: Participate in the acute general medical / geriatric on-call rota in accordance with the rostered commitments under the POCC 2023.
Clinical Leadership & Governance: Direct multidisciplinary team meetings; participate in clinical audit, incident reporting, risk management, and service development initiatives in line with HSE national standards.
Teaching & Training: Engage in the education, training, and continuous assessment of undergraduate medical students and postgraduate trainees (RCPI / BST / HST), as well as interprofessional teaching with nursing and allied health professionals
